River Valley Health & Dental Center is committed to providing comprehensive and integrated primary care to the community wee serve.  This includes an on-site behavioral health and social services team comprised of Licensed Clinical Social Workers (LCSW) and a Community Navigator.  Our social services department is essential to the center as a whole and serves as an asset that allows River Valley Health & Dental Center to stand apart from many other primary care facilities.

Primary care is a first-line of intervention for any number of needs experienced by patients across their lifespan.  Our LCSWs work side-by-side with their medical staff colleagues to integrate physical, behavioral and mental health care within their visit.  Through a warm hand-off process, our LCSWs complete assessments, offer brief therapeutic intervention and facilitate referrals for any individual struggling to cope with an acute or ongoing emotional, behavioral or substance misuse concern.  Individuals are additionally welcome to be seen on a walk-in basis when experiencing an acute need or crisis.

Supplemental to clinical behavioral health services, our Community Navigator services works to minimize barriers that may stand in the way of routine healthcare.  Whether it's helping a patient access needed resources in the center, community or finding options to meet their basic living needs, or assisting with a variety of referrals, the Community Navigator assures that each patient is treated with compassion and respect while receiving support to work towards their individual goals.
